Systemic Formulas Patient Portal offers you an efficient way for your patients to order online without the need for inventory or additional training, offering easy access for your patients to view Systemic Formulas products, place their own orders, and have their orders shipped directly to their door. This eliminates the need for you to keep inventory, handle sales tax, and calculate shipping costs. Everything is done on our end to simplify the experience with Systemic Formulas for both you and your patients.

How do I get started?
To create a Patient Portal account you will need to fill out three forms to receive your doctor code. The first is the Systemic Formulas Independent Seller Agreement. This agreement will provide an in-depth explanation of your commissions from portal sales and details on when you will receive commission payments. The second form needed to set up your portal account is a W-9 form. Finally, the third form is where you will indicate how you would like to receive your Patient Portal commissions, and provide the necessary information to receive your commission payments.
After we have received your completed Systemic Formulas Independent Seller Agreement, W-9 form, and have submitted your commissions payment method selection, you will receive an email in 1-2 business days. This will include your doctor code as well as instructions on how your patients will be able to set up their account using your doctor code and place their orders online.
